Website: "Dr. With its Immune Balancing Protocol, Couvaras is systematically trying to reduce the sources of inflammation. It is important in so-called unexplained infertility cases to investigate immune system imbalances before proceeding directly to IVF, as is the typical strategy of most other reproductive endocrinologists."
Patient Comment: “Dr. Couvaras is the BEST. It's brilliant/unorthodox. He thinks outside the box. What other doctors call "unexplained infertility," he likes to call it... "that's bullshit, there's no unexplained cause, there's always an explanation, people just don't want to hear it." He is normal, balanced and willing to try different things. I think you have to be willing to think outside the box because if you don't, you probably won't get pregnant."

Doctor's comment: “Immune problems are treated with immunotherapy. If the cause is related to activated immune cells and their cytokines, treatments include: intravenous immunoglobulin (IVIg) and intralipids. When acquired or inherited thrombophilia causes clotting of the placental vessel and subsequent pregnancy loss, heparin and aspirin are the treatments of choice. When blood clotting is the result of an immunological process, steroids can be used."

Patient Comment: “...I went to 6 before I had a great Dr. who absolutely believes in and treats immune problems. He doesn't advertise. Everyone who comes to him has heard of him from someone who has been successfully treated. The only thing it doesn't do is LIT (since it's not legally possible). He did LIT for many years before it was banned. His name is Robert Friedland and he's in Beverly Hills (but he doesn't charge prices in Bev Hills). I have met numerous women in his waiting room who have had 6+ m/c and eventually had a baby after he treated them. The only reason I'm posting this is because it took me a long time to find someone who genuinely believes in my problems. It took me 2 years to find him and I wish I had known about him sooner. Now he's supporting me with Dr. Beers LIT in Mexico.”

He is very helpful in trying to get Dr. Getting Beermeds through his office. It's practical, but not in a harsh way. He is very concerned about the well-being of his patients and takes it almost personally when a Pg fails. Best of all, my Dh thinks it's the best, which helps a lot. He is amazing!"

“Our work for patients with recurrent pregnancy loss and failed implantation cycles during IVF includes testing for APLS (lupus anticoagulant, B2GP-1 Ab and anticardiolipin Ab). We will test typical blood clotting agents when indicated and these include: Leiden V (or activated protein C resistance as an initial screening test), prothrombin II gene mutation. MTHFR is no longer considered part of the required testing for RPL or failed implant cycles as the literature does not support this test. I would rather rely on a fasting homocysteine level. We will look at Protein S, C and Antithrombin III IF the patient's coagulation history supports these tests, but again these are not part of the panel required for patients with RPL or implant failure. We did not analyze NK activity (uterine or peripheral) as the literature shows at best conflicting evidence and/or no benefit in other studies. We also usually get thyroid antibodies (TPO) in this population. We don't have specific reproductive immunologists that we typically work with, but I do have a few experts in the field of RPL and failed implantation whose advice I often seek in difficult cases."

This clinic is up to date with the latest tests and treatments for immune-related pregnancy failure and has just introduced a Systemic Inflammatory Fertility Test (SIFT). This expanded assessment examines several key factors in the circulating blood that are known to trigger inflammation. An individual plan is then formulated to improve the patient's health and reproductive success."

From the website: “Dr. Known for the dedicated care she provides to her patients, Coulam has an international reputation for her contributions to medical/scientific education and research. As one of the first female reproductive immunologists in the field, she also established other "firsts" while raising ten children of her own. He has over 200 publications in peer-reviewed journals to his credit. Miss Dr. Coulam has particular expertise in the field of recurrent miscarriage and is an internationally sought-after speaker on this topic as well as topics related to reproductive immunology.” From www.winfertility.com.

Comment:"Dr. Sher is an internationally recognized authority on the immunological causes of infertility and the pioneer of many "firsts" in the field of IVF. 60% of his patients travel from out of town to see him; that is their experience and success rate.”
From the website: “The idea that after failed IVF, trying the same prescription over and over again is all that is needed for eventual success is too simplistic. There are numerous non-embryological factors that can be responsible for IVF failure. This section addresses the immunological factors that cause implantation failure. There are two types of immune responses or immunological factors involved in conception and infertility: 1. Autoimmune diseases are more common and are involved in more than 90% of infertility cases related to the immune system... This is an abnormal response that is not associated with various diseases related to pregnancy. . 2. Alloimmune diseases, in which antibodies are produced against tissues of the male partner (e.g. paternal sperm proteins). Alloimmune problems are associated with less than 10% of implantation failures or recurrent miscarriages.”

One of the top fertility doctors in the United States. It offers testing for immune issues "before couples opt for costly and potentially unnecessary IVF treatment."
From the website: “Laboratory tests may be ordered to assess the possibility of an immune response related to your history of pregnancy loss. The field of reproductive immunology is a relatively new area of medicine. Our goal is to identify possible explanations for recurrent miscarriages and to offer individualized medical treatment options."

My husband and I decided to drive from MD to NYC for a special test through this doctor's office. Meeting with him further confirmed that according to Dr. Sheri Hamersley were on the right track. Both doctors took excellent care of us and my next two pregnancies were healthy pregnancies that ended on time after multiple miscarriages.

“We provide research and treatment services for recurrent miscarriages. We test for all recognized and significant causes of recarpeting-to-loss-salute antibodies. We can also test for natural killer cells and do so where appropriate. I will work with anyone who can help my patients have a baby. Recurring miscarriages really suck and I spend 20-25% of my time fighting it."

I was delighted to hear the name of Dr. Beer in my files and looked forward to speaking to him about my situation. He had nothing but praise for the work and research that Dr. Beer has done/does. That was a great relief for me. His practice is professional and his nurse is very helpful and friendly. I even had blood drawn for my NK tests. I had to do another IVIG (back to back) because my NKs were slightly elevated. As soon as I left his office, Dr. Parnell immediately Dr. Beer and spoke to him about my treatment. She's also very open to the idea that immunology could play a role, and is happy to write instructions for you to have your APA and ANA levels tested, usually by RIA Laboratories in Los Angeles. Anything beyond the need for Lovenox + Prednisone and you have to be in charge. When I gave her information from Dr. Beer's book and requested a request for additional testing, she was happy to oblige, but acknowledged that I have a lot more research on reproductive immunology than she does."

: "If the immune system is the cause of the miscarriage, the probability of a successful pregnancy of the mother after 3 miscarriages without treatment such as Intralipid or/and IVIG is 30%, after 4 miscarriages 25% and after 5 miscarriages only 5%. With proper treatment, the overall success rate is 80%. We are a clinic specializing in treating women with recurrent miscarriages, recurrent implantation failures and immune-related infertility. We offer treatment adapted to the individual case of the woman with intralipids, intravenous immunoglobulin (IVIG) and advanced operations of hysteroscopy and laparoscopy.”

“She will test for NK cells and prescribe prednisone if necessary. You don't have to do IVF to see it. He also said that due to my history of recurrent miscarriages I would use prednisone, although my immunoassays were low, nothing out of the ordinary. : “One of the most overlooked diagnoses for recurrent implantation failure and recurrent miscarriages in the early stages is an underlying dysfunction of the female immune system. Although this concept of hyperactive NK cells is widely accepted by most clinicians, the method used to diagnose and treat these disorders is controversial. As a result, research and treatments at this point must be considered experimental. These controversial empiric therapies include intralipids or IVIG infusions, immune modifiers such as Humira, and related drugs. To try to identify the cause of repeated pregnancy losses, genetic testing is done on both couples, sperm testing, clotting assessment, and immune disorders.

Comment:“I will perform LIT according to the following protocol (his words)
Come to us in the morning (ideally between 8:00 and 9:00 a.m.) as we have the syringe in our laboratory at certain times. The first step would be to draw blood (70ml) from your partner. The treatment is usually finished by 12:00 p.m. The therapy costs are 230.58 euros + 68.05 euros if anti-D prophylaxis is indicated. Side effects are primarily local redness and itching for around 2-3 weeks. The risks are those of infectious diseases (as in transfusion medicine) so your partner should be tested accordingly. HLA antibodies are formed, antibodies against other blood cells could possibly be formed. But in our 20 years of experience we have not had any negative reports about it.”

“Studies continue to show that the complex communication between the implanting embryo and the mother's immune system is essential for a successful pregnancy. Problems that cause miscarriages or repeated chemical pregnancies can also lead to recurrent implantation failure if they occur early in pregnancy. In these cases we do a thorough screening for uterine, sperm, infection and where possible genetic issues, but we will also try to find and address any immune or blood clotting issues."

He scheduled an endometrial biopsy for me back on December 1st, prior to the Beer Center's instructions. If your own doctor is willing to help you draw blood, you can pack them yourself and send them by Fedex. Many women in the UK have had their blood tested this way. CARE Nottingham Mandy mentioned they have a good reputation so there is a 12 week waiting period for a consultation, you also need a referral from your own doctor. Like Mandy said there are a couple of options, it's a personal choice. I choose Trevor Wing to do the immunological test for now because it's only a half hour drive away.

FAQs
How do I find a collaborating physician in PA? ›
If you work for a hospital system or an office that has multiple providers, asking a physician you already may know could be an easy option for an agreement. Also ask PAs in your network if they have any physicians they work with who are PA-friendly and may be open to additional collaboration.
What is a collaborating physician? ›Collaborating physician provides direction and oversight and must be available to the NP by radio, telephone, or telecommunications, and must be available for consultation or referrals from the NP (Ala. Admin.
Is a collaborating physician liable for a nurse practitioners error? ›An NP owns a practice and engages a physician to be the collaborator, for state law purposes. Here, the physician takes on minimal liability in return for whatever the NP is paying the physician. A plaintiff is unlikely to sue the physician collaborator and probably won't even know who the physician collaborator is.
How many nurse practitioners can a physician supervise in Alabama? ›NPs practicing in Alabama are required to have a written collaborative practice agreement with a physician. One physician may collaborate with no more than three full-time nurse practitioners, or 120 NP collaboration hours per week.
What states allow PA to practice independently? ›Alaska, Arizona, and Wyoming all allow PAs to provide patient care in a modernized practice environment. These states have five of the six key elements.
Can a PA work alone independent of a physician )? ›PAs are dependent practitioners working with a dedicated consultant or GP supervisor, but are able to work autonomously with appropriate support. Supervision of a qualified PA is similar to that of a doctor in training or trust grade doctor, in that the PA is responsible for their actions and decisions.
What are the 4 types of collaboration? ›As follows is a brief description of the four main types of collaboration known to us today (Alliances, Portfolios, Innovation Networks, and Ecosystems), and the importance of each.
What are the 3 liabilities of a physician? ›95/ 2006 on healthcare reform in the medical malpractice domain stipulates that medical staff can be held accountable in the following forms: disciplinary liability, administrative liability, civil liability and criminal liability.
What can a nurse practitioner not do that a doctor can? ›They then go through additional education and training to become licensed to do more throughout the treatment process. Essentially, a nurse practitioner can do everything a doctor can do, although they are not able to perform surgical procedures.
What are the 4 elements needed to prove malpractice in nursing? ›To do so, four legal elements must be proven: (1) a professional duty owed to the patient; (2) breach of such duty; (3) injury caused by the breach; and (4) resulting damages.
Can a NP own their own practice in Alabama? ›
Yes. The rules governing Collaborative Practice in AL do not prohibit a CRNP or CNM from owning a practice. However, CRNPs and CNMs who desire to practice are required to enter into a Collaborative Practice Agreement with a physician licensed in the state of Alabama.
Who has more authority a PA or NP? ›The primary difference is in the level of supervision, depending on which state they practice in. Nurse practitioners can practice independently in about half of the country and have the authority to prescribe medication in those states. Physician assistants always work under the supervision of a doctor.
How many patients should a NP see in a day? ›Generally, an NP needs to see 20 patients a day to generate enough money to make a practice profitable. However, that number depends on the fee schedule, the NP's salary and benefits, the practice's overhead expenses, and the practice's expectation of profit.
Will PA ever have autonomy? ›The rules proposed in the fourth phase, which shift PA practice from a supervision model to a collaboration model by the end of 2023, are set to become effective on July 15, 2022.
Does PA have full practice authority? ›Full practice authority for PAs should be based on training, clinical experience, and competency. Rather than adjusting the previously established threshold to obtain full practice authority to meet current PA standards, PAs should pursue further training and certification to earn this privilege.
Can you practice as a PA without a license? ›Practicing as a physician assistant without a renewed license may lead to disciplinary action against your license. There is usually no renewal grace period. Licenses not renewed by the expiration date are considered "delinquent."
Who is more independent NP or PA? ›An NP has more independence, whereas a PA works interdependently with a doctor. Sometimes there is a comparison between a nurse Practitioner vs. a doctor. In some states, a nurse practitioner can work independently without any direct physician oversight.
Why be a physician assistant and not a doctor? ›Becoming a physician assistant allows someone to provide health care without the lengthy and strenuous education that is mandatory for a doctor. Physician assistants can examine patients, prescribe medicine, order diagnostic tests and perform a host of other duties that doctors also do, experts say.
What is something a doctor but not a PA can do? ›Another major difference between an MD and a PA is that doctors can legally practice independently while a PA always requires supervision. Both can do the following: Diagnose and treat patients.

What are the 4 C's of malpractice? ›Recognizing that you are an imperfect human being who will make mistakes, you can nevertheless reduce your risk of causing harm, and of being sued successfully. Start by practicing good risk management, building on the old adage of four Cs: compassion, communication, competence and charting.
How many states can NP work independently? ›States Where NPs Can Practice Independently. As of August 2021, 24 states and the District of Columbia have approved NP practice autonomy. Note that state regulations change frequently, and advanced practice nurses should check with state nursing boards for the most current regulations.

Do doctors prefer NP or PA? ›Rarely is there a single right answer for whether a nurse practitioner or physician assistant is better for your practice. Rather, you'll have to consider the tasks you want them to do, what your goal is in hiring them, and other factors, like state laws and tradition in your healthcare provider network.
Who is more educated a PA or NP? ›
NP vs PA Education Requirements & Programs
Program length is roughly the same for both a PA and an NP after earning a bachelor's degree. Both are master's level programs and can be completed in 2-3 years depending on the specific program.
NPs and PAs: More Than Just Health Care Providers

How many nurse practitioners can a physician collaborate with in Pennsylvania? ›Section 21.287 provided that a physician could not supervise more than four CRNPs who prescribe and dispense drugs at any one time.
Is Pennsylvania a collaborative state? ›Pennsylvania is considered a Restricted Practice state due to the requirement for a collaborative agreement to practice and a collaborative agreement to prescribe. Besides the two collaborative agreements, PA requires two separate licenses: a CRNP License. a Prescriptive Authority License.
What can a PA do that a NP Cannot? ›Scope of medical treatment
A physician assistant can diagnose and treat a wide array of medical concerns, prescribe medicine, and may even perform surgery. A nurse practitioner can help diagnose and treat patients with routine and complex medical conditions, sometimes working alongside physicians to do so.
FULL: NPs can prescribe, diagnose, and treat patients without physician oversight. Nurse practitioners who operate in full-practice states are also allowed to establish and operate their own independent practices in the same way physicians do.

How much do FNP make in Pennsylvania? ›Many people have primary care providers that are NPs or PAs. But what's the difference? In basic terms, a nurse practitioner is a registered nurse (RN) with advanced education and clinical training. A physician assistant is a medical professional with advanced education who is trained in the same way physicians are.
Do patients prefer NP or PA? ›Both groups of respondents were equally likely to mention previous experience with their provider type as a reason for their preference (prefer physician = 19%; prefer NP/PA = 21%). Conclusions: Provider qualifications and previous health care experiences are cited as key reasons for preferring all provider types.
